1995 AICHE Thiels Failures in Urea Reactor Vessels
Several case histories of urea reactor vessel failures are discussed, as well as remedial measures taken to restore them to a level suitable for continued service. The failures were produced by a variety of f actors including defects introduced during manufacturing or fabrication, service-related deterioration, and/or upset operating conditions. These incidents can cause not only life threatening injuries or even fatalities, but also prolonged periods of business interruption.
